Factors associated with non-adherence to antiretroviral treatment among people living with HIV monitored in health centers in the Ouabri region, Burkina Faso

The WHO's “test and treat” strategy has helped increase the number of people living with HIV on antiretroviral drugs. However, the goal of neutralizing plasma viral load is far from being achieved. Adherence, one of the keys to this neutralization, remains a challenge. The objective of our study was to identify the factors associated with non-adherence to antiretroviral treatment in the Oubri region, one of the seventeen regions of Burkina Faso. We conducted an analytical study from June to December 2024. People living with HIV aged 18 years or older were interviewed as they came in for their follow-up visits. Adherence was calculated based on their reports of medication use. People who had missed at least one day of medication were considered non-adherent. A logistic regression at the 5% threshold allowed us to identify the associated factors. The average age of the participants was 45.89± 12.04. There were 232 women (70.73%) and 96 men (29.27%). The proportion of non-adherent individuals was 20%. The factors independently associated with non-compliance were difficulties in traveling to the health center due to distance (OR 4.23, 95% CI [2.24-8]) and perception of the severity of HIV-related illness (OR 1.97, 95% CI [1.1-3.56]). It is necessary to bring follow-up centers closer to the population, and community adherence support strategies are recommended.
